The following forms need to be completed and returned to the camp office before June 1, 2013.

Required for Attendance
Every camper must have had a physical exam within the last two years prior to attending camp. The health exam form must be completed by your daughter's physician.
Required for Campers with Medications*
Authorization for the Administration of Medication Form
*This form needs to be completed and signed by a doctor for every medication (prescription and over the counter, including vitamins, eyedrops and creams) your daughter brings to camp.

The following forms are required for campers registered for the Senior Sessions. These forms are also due June 1st.
