Every camper must have had a physical exam within the last two years prior to attending camp. The health exam form must be completed by your daughter's physician.
Authorization for the Administration of Medication Form
This form needs to be completed and signed by a doctor for every medication (prescription and OTC) your daughter brings to camp.
