Parents must complete this form.  This information is confidential to Camp Directors, Nurses and first aiders.  It will be shared with appropiate staff if deemed to be in the camper's best interest by the Camp Nurse.  Parents must complete this Form and return to camp by June 1st.

This form lets parents indicate which over the counter medications they DO NOT wish their daughter to be given on an "as needed" basis. All medications that are not on this list whether over the counter or prescription must be accompanied by Authorization for the Administration of Medications Form signed by authorized prescriber and parent or it cannot be given at camp. Parents must complete this Form and return it to camp by June 1st.

Connecticut State Law and Regulations require an authorized prescriber (M.D., P.A, APRN) or dentist’s written order and parent or guardian’s authorization for a nurse or camp personnel with current Medication Administration Training to administer medications. Authorized Prescriber and Parents must complete this Form and return it to camp by June 1st.

This Youth Camp Health Exam/Record is required for all Campers and Staff. Physical exams are valid for 2 years from date of last examination. Physcian must complete this Form and Parents must return it to camp by June 1st.
